St. Peters Health Partners, is seeking a full-time Advanced Practitioner to join Palliative Care Partners, a service line across St. Peter’s Health Partners supporting patients and their families who are dealing with a serious illness. The Practitioner will provide palliative care services in the community / home settings including home visits, tele-medicine, on-call rotation, and collaboration with interdisciplinary team.

This role will work closely with an interdisciplinary team including an MD, Social Worker, and Spiritual Care support to provide comprehensive care to patients with advancing illness primarily in the community setting. The Advanced Practitioner will work with consultant and practice staff to manage pain and symptoms, address goals of care and treatment plans and provide guidance and education to patients and families.

  • Work schedule is Monday-Friday
  • Preference will be given to those candidates who are board certified in Hospice and Palliative Care.
  • Two years of clinical experience, preferably in palliative care and/or home health preferred.
  • Travel will be required for the outpatient role.

REQUIREMENTS

Applicants should have the following:

  • Board Certification, NYS licensure and DEA required
  • Applicants should possess excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented

RECRUITMENT PACKAGE:

St. Peter’s Health Partners Medical Associates, P.C. offers a comprehensive salary and compensation package that includes:

  • Competitive salary commensurate with experience
  • Base compensation is between $127,030 and $137,192.
  • The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above.
